29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  a) Roof: On 07/09/2024, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that no protective eyewear was provided to employees operating these tools at 130 PSI.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  a) Roof: On 07/09/2024, the employer exposed employees to fall hazards of approximately 24-feet to the lower ground level, in that fall protection systems were not provided to employees while engaged in roofing activities on a steep 10/12 pitched roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Extension ladder on front side of structure: On 07/09/2024, the employer exposed employees to fall hazards of  24-feet to the lower ground level, in that the Werner 32-feet extension ladder was not extended at least 3-feet past the upper landing or secured to prevent movement during use.
