OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor: a) G Building, 121 Halbritter Drive, Altoona, PA 16601: Observed on or about June 27, 2024, industrial storage racks located in the G Building were not anchored to the floor. The industrial storage racks were at risk of being tipped over by powered industrial trucks used to load and unload materials from the racks. Employees working in the G Building were exposed to struck-by and crushed-by hazards.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a) Main Warehouse, 121 Halbritter Drive, Altoona, PA 16601: Observed on or about June 27, 2024, access to three emergency exit doors located on the north side of the main warehouse were obstructed by sandbags which were located directly in front of the emergency exit doors. Employees working in the main warehouse were exposed to a fire hazard. b) G Building, 121 Halbritter Drive, Altoona, PA 16601: Observed on or about June 27, 2024, access to an emergency exit door was obstructed by a pallet and multiple boxes which were stored directly in front of the emergency exit door. Employees working in the G building were exposed to a fire hazard.

Both flexible cords were being used as a substitute for the fixed wiring of the structure.
