29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) 23-25 Hoss's Drive, Lewistown, PA, Flat Roof - Employees were framing a new parapet on a roof approximately 13 feet above grade on a flat roof without a fall protection system in place, on or about July 11, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.454(c)(3):The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency: (a) 23-25 Hoss's Drive, Lewistown, PA, Flat Roof - Employees utilized an elevated platform which was attached to the boom of a JCB 507-42 telehandler rental. The platform and components were not capable of supporting loads as required in that the platform width was greater than the width of the truck (measured across the load bearing tires, plus 10 inches on either side), on or about July 11, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
