Location: 329 50th Street, Brooklyn, NY  a) On or about 7/10/2024, an employee fell from the tubular welded frame scaffold onto the sidewalk shed. The employer did not initiate an inspection program which provides for frequent and regular inspections of the scaffold to be made by a competent person.

Location: 329 50th Street, Brooklyn, NY  a) On or about 7/10/2024, an employee worked from a tubular welded frame scaffold that was not fully planked. The employee fell approximately 18 feet to the sidewalk shed.

Location: 329 50th Street, Brooklyn, NY  a) On or about 7/10/2024, an employee worked from a tubular welded frame scaffold without using any form of fall protection. The employee fell approximately 18 feet to the sidewalk shed. The employer did not provide training to an employee who worked from a scaffold. Employee could not recognize hazards associated with scaffold work.
