Employees were exposed to struck-by hazards associated with an unsecured liquified petroleum gas cylinder.  b.  On or about July 12, 2024, in the charging area, the employer did not ensure the Clark forklift, Model 30L, was removed from service during a time in which it was not in safe operating condition. Employees were exposed to struck-by hazards associated with an unsecured liquefied petroleum gas cylinder and unreadable name plate.
