29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: (a) On July 12, 2024, at or near 19th Street Northwest and Central Avenue North in Beulah, North Dakota, an employee was standing on a 36-by-36-inch wooden pallet that was being used as a scaffold/work platform that was not designed by a qualified person�and had been placed on the forks of a JLG SkyTrak 6042 Telehandler to raise and lower the employee.

29 CFR 1926.451(c)(2)(v): Forklift was used to support scaffold platforms without ensuring the entire platform was attached to the fork: (a) On July 12, 2024, at or near the corner of 19th Street Northwest and Central Avenue North in Beulah, ND, an employee was exposed to fall hazards greater than 6 feet while working from a wooden pallet elevated on the forks of an unmanned JLG SkyTrak 6042 Telehandler. The platform was not fully secured to the Telehandler and/or the forks.
