29 CFR 1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, was not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. At 406 E Industrial Drive, Gerald, MO, 63037, on July 11, 2024 and at times prior thereto, employees were exposed to amputation hazards from a foot activated shear which was not guarded to prevent body parts from entering the work area.

29 CFR 1910.147(c)(6)(ii):The employer did not certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. At 406 E Industrial Drive, Gerald, MO, 63037, on July 11, 2024 and at times prior thereto, employees were exposed to the dangers of the unexpected energization of equipment due to non certified inspections of Lock out/ Tag out procedures that did not include authorized employees and have the name of the person performing the inspection.
