29 CFR 1910.141(a)(5): Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected.  In the exam rooms and in the break room at 471 Center St. Ludlow MA: On July 17, 2024, and at times prior, the employer did not ensure that an effective extermination program was established when mice activity was discovered in the facility.  Rodent droppings were observed in the break room on plastic storage containers and in garment drawers in patient exam rooms.
