Main Street Leesburg, FL 34748; On or about 7/16/24, the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to struck-by and crushing hazards while attempting to fix a hydraulic system on Uline Hydraulic Lift Table without training on safe practices and procedures.
