29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite located at 1701 Market Street, Philadelphia, PA 19103 - On or about July 18, 2024, the employer did not ensure employees engaged in commercial construction activities 6 feet or more above lower levels were protected guardrail systems, safety net system, or personal fall arrest system. Employees were setting up a suspension scaffold on top of an 18-story building without the use of fall protection, thereby exposing employees to falls of approximately 220 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite located at 1701 Market Street, Philadelphia, PA 19103 - On or about July 18, 2024, the employer did not provide effective fall protection training to employees who were exposed to fall hazards. Employees were setting up a suspension scaffold on top of an 18-story building without being provided effective fall protection training, thereby exposing employees to fall hazards.
