The pole that was being used to support the scaffold was resting directly on the stone driveway, not fully supported, exposing the employee(s) to fall hazards of approximately 15 feet to the ground below, on or about June 13, 2024. (b) Brandywine Drive, State College, PA 16801, Rear of House, - Employee(s) performed siding work from a pump jack scaffold system. The pole that was being used to support the scaffold was resting directly on uneven dirt, not fully supported, exposing the employee(s) to fall hazards of approximately 5 feet to the ground below, on or about June 13, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) Brandywine Drive, State College, PA 16801, Front and Rear of House - Employees performed work from two pump jack scaffold systems, and the scaffolds was not erected under the supervision of a competent person, issues included lack of fall protection, missing mud sills, scaffold poles were out of plumb, and scaffold poles that were not tied into the structure properly, exposing the employees to serious injury, on or about June 13, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
