29 CFR  1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:  24 Perkins Row Wethersfield, CT. On or about July 18, 2024 employees were not wearing appropriate personal protective equipment under the following conditions while conducting roofing activities: a) No head protection worn by employees working on the ground level and exposed to falling object hazards from those working on the roof above them. b) No eye protection worn by employees exposed to flying object hazards from the use of using pneumatic nailers, hammers, and cordless electric drill/ drivers.

29 CFR  1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  24 Perkins Row Wethersfield, CT. On or about July 18, 2024 a 16-foot portable extension ladder used to access a roof was used while not level. The ladder was canted approximately 10 degrees to the right with the left foot of the ladder off the ground.

29 CFR  1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear:  24 Perkins Row Wethersfield, CT. On or about July 18, 2024 the base of a 16-foot portable extension ladder was partially obstructed by a compressed air hose, dunnage, and several extension cords.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   24 Perkins Row Wethersfield, CT. On or about July 18, 2024, employees engaged in framing activities on a 10-12 pitch roof at heights varying from 8.5 to 25 feet above ground level were not protected by any guardrail systems, personal fall arrest, or alternative fall protection measures.  AGL General Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1666505, citation number 1, item number 1, and was affirmed as a final order on July 6, 2023, with respect to a workplace located at 34 Pebble Road Wethersfield, CT 06109.
