29 CFR 1926.652(g)(1)(iii):Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields: a) On July 17th, 2024, employee installing sewage and water lines in an excavation measuring approximately 9.5 feet deep, used an extension ladder which was positioned outside of the trench box, to enter and exit the excavation.
