At a residential worksite located at 5510 South 4th St., St. Joseph MO -- Employees were engaged in roofing duties using pneumatic nail guns without eye protection. This exposed employees to getting debris in their eyes causing injury. This was documented on or about 7/17/2024.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a residential worksite located at 5510 South 4th St, St. Joseph MO -- employees engaged in roofing duties were using an extension ladder to gain access to the roof. The extension ladder was not extended 3 feet above the landing surface. This exposed the employees the employees to fall hazards of approximately 10 feet to ground level. This was documented on or about 7/17/2024.
