29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 5510 South 4th St., St. Joseph MO -- Employees were engaged in roofing duties using pneumatic nail guns without eye protection. This exposed employees to getting debris in their eyes causing injury. This was documented on or about 7/17/2024.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a residential worksite located at 5510 South 4th St., St. Joseph MO -- employees engaged in roofing duties were not using any personal fall arrest system. This exposed the employees to fall hazards of 12 plus feet to ground level. This was documented on or about 7/17/2024.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a residential worksite located at 5510 South 4th St, St. Joseph MO -- employees engaged in roofing duties were using an extension ladder to gain access to the roof. The extension ladder was not extended 3 feet above the landing surface. This exposed the employees the employees to fall hazards of approximately 10 feet to ground level. This was documented on or about 7/17/2024.
