29 CFR 1910.1053(j)(1): The employer did not include respirable crystalline silica in the program established to comply with the hazard communication standard (HCS) (29 CFR 1910.1200): On, or about July 19, 2024, the employer did not provide training to the employees on the hazardous chemicals, present at the shop such as, but not limited to, crystalline silica generated while fabricating countertops consisting of granite, marble and other silica containing stone. GQM Stone Solutions, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.1052(i)(1), which was contained in OSHA inspection number 1663093, citation number 1, item number 7a and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 2260 Elmhurst Road, Elk Grove Village, IL 60007. In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstration that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1053(g)(2): Where respirator use was required by this section, the employer did not institute a respiratory protection program in accordance with 29 CFR 1910.134: On, or about July 19, 2024, the employer failed to establish and implement a written respiratory protection program when employees were required to wear 3M 6000 series elastomeric half face respirators with P100 filter cartridges and 3M, N95, filtering face-piece respirators, while fabricating countertops consisting of granite, marble and other silica containing stones. GQM Stone Solutions, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.134(c)(1), which was contained in OSHA inspection number 1663093, citation number 1, item number 2b and was affirmed as a final order on October 25, 2023, with respect to a workplace located at 2260 Elmhurst Road, Elk Grove Village, IL 60007.
