Dupont Highway New Castle, DE 19720: On or about July 18, 2024, employees accessed the scaffolding system for approximately 2 weeks that contained multiple hazards including but not limited to bent/broken parts, modified pieces not authorized by the manufacturer, and portions of the scaffold not fully supported.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a) Chik-Fil-A construction site in the parking lot of 4008 N. Dupont Highway New Castle, DE 19720: On or about July 18, 2024, CSHO observed employees performing brick facade installation while working from a frame scaffold approximately 20 foot with damaged/modified components (not authorized by the manufacturer).
