On or about July 19, 2024, at the above addressed jobsite, an employee conducting painting operations from a tubular welded framed scaffold, twenty-five (25') feet above the ground, was exposed to fall hazards due to the scaffold leg(s) did not bear on a mud sill.

On or about July 19, 2024, at the above addressed jobsite, an employee conducting paint trimming operations from a tubular welded framed scaffold, twenty-five (25') feet above the ground, was exposed to fall hazards due to the lack of a complete guardrail system.

On or about July 19, 2024, at the above addressed jobsite, employees performing residential roofing operations, thirteen (13') feet two (2") inch above the ground, were not protected from falls, thereby exposing the employee to fall hazards.
