29 CFR  1926.501(b)(4):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  Location: 38 Jackson St. Hoboken NJ 07030  a: The employer failed to ensure that employees were protected from falls through holes. An employee engaged in masonry work suffered back injuries after falling through a 5'x4' elevator shaft from the 6th onto the 5th floor of the building.   Violation occurred on or about July 19, 2024.
