1910.147(d)(2)  The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) (IDENTIFY MACHINERY OR EQUIPMENT)  a)	Raw Side Area - On or about June 24, 2024, an employee was exposed to machine hazards associated with rotating parts during removal of a discarded chicken part 011 the Line #4 Hopper Conveyor model #327 BB, serial #8020, equipment. The employer failed to ensure that the equipment was shutdown for locking out prior to removing the discarded part.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification.
