OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Location: Lumber Yard On or about July 19, 2024, employees working around industrial steel storage racks were not protected from struck by and crush by hazards where the employer did not identify and post the capacities of the steel storage racks, install the racking indoors on a concrete floor, and anchor the steel storage racks to the floor.

Location: Warehouse upper storage On or about July 19, 2024, employees were exposed to trip and fall hazards where an approximately 1/2" change in flooring elevation was present at the entrance to the storage racks and was unmarked or ramped.
