29 CFR  1910.332(b)(1): Employees had not been trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments.  On or about July 15, 2024, GC Industrial Services did not ensure that each employee, who was reasonably expected to face a comparable risk of injury due to electric shock or other electrical hazards, was trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignment of cleaning utilization equipment. Three employees were exposed to electrical hazards including but not limited to electric shock while cleaning a YORK air handler unit on the rooftop of IRCA Group USA LLC's North Building at 8021 New Hampshire Avenue in Affton, Missouri. Employees were not trained in and familiar with the following safety-related work practices, for locking and tagging out utilization equipment as well as working inside of, on, or near electrical conductors and equipment where electrical hazards were present:  A) For checking stored energy after deenergizing equipment, stored electrical energy shall be released, capacitors shall be discharged, and high capacitance elements shall be short-circuited and grounded. Otherwise, if the capacitors or associated equipment are handled in meeting this requirement, they shall be treated as energized.  B) For verification of isolated electrical energy, a qualified person shall operate the equipment controls or otherwise verify that the equipment cannot be restarted.  C) For verification of isolated electrical energy, a qualified person shall use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately before and immediately after the test.  D) For working where exposure to energized parts may occur, conductive apparel (e.g. glasses, jewelry, watch bands, key chains, conductive thread, etc.) may not be worn unless it is rendered nonconductive by covering, wrapping, or other insulating means.  E) For utilizing test instruments and equipment, only qualified persons may perform testing work on electric circuits or equipment. The test instruments and equipment shall be visually inspected before use, and the test instruments and equipment shall be rated for the circuits and equipment to which they will be connected as well as designed for the environment in which they will be used.

29 CFR  1910.333(b)(2): Circuits energizing the parts of fixed electrical equipment had not been locked out or tagged out or both in accordance with the requirements of this paragraph.  On or about July 15, GC Industrial Services did not ensure that the circuits energizing fixed utilization equipment were locked out or tagged out, or both in accordance with paragraph (b) of section 1910.333. Three employees were exposed to electrical hazards including but not limited to electric shock and burns from cleaning a YORK AHU that was not locked or tagged out.

29 CFR  1910.333(b)(2)(i): The employer had not maintained a written copy of the procedures outlined in paragraph (b)(2) and had not made it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives.  On or about July 15, 2024, GC Industrial Services did not ensure that a lockout taggout procedure for utilization equipment comprised of a copy of paragraph (b)(2) of this section or that the procedure complied with paragraphs (c) through (f) of 1910.147 and addressed the electrical safety hazards covered by Subpart S, as well as incorporated the requirements for having a qualified person use test equipment to test the circuit elements and electrical parts of the equipment, which employees would be exposed to, in order to verify that the circuit elements and equipment parts were energized. Three employees were exposed to electrical hazards while cleaning a YORK AHU from not having and utilizing a lockout taggout procedure for the control of hazardous energy of a YORK AHU at IRCA Group USA LLC's North Building at 8021 New Hampshire Avenue in Affton, Missouri.
