29 CFR  1910.134(f)(2):Employee(s) using tight-fitting facepiece respirators were not fit-tested prior to initial use of the respirator or annually refitted:  (a) 860 Pennsylvania Blvd., Feasterville Trevose Pa., 19053- On or about July 23, 2024, Polish and Fabrication area, employee(s) in the Polish and Fabrication area were required to wear 3M half-face respirators to protect from inhalation hazards. The employer did not provide a fit-test prior to initial use of the respirator.

The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator:  (a) 860 Pennsylvania Blvd., Feasterville Trevose Pa., 19053- On or about July 23, 2024, Polish and Fabrication area, the employer did not provide a medical evaluation for the employee(s) required to wear a half-mask respirator during polishing of marble, granite, quartz and quartzite stone countertops, exposing employee(s) to a respiratory hazard.

29 CFR  1910.178(q)(6):Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts, except as provided in paragraph (q)(12) of this section. Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer:  (a) 860 Pennsylvania Blvd., Feasterville Trevose Pa., 19053- On or about July 23, 2024, Polish and Fabrication area, a Doosan Pro5 orange in color forklift - Model 630p-5, serial number FGADC-1790-02422 rear flashing light covered with cardboard. The flashing light is a warning device to alert pedestrians and other personnel of forklift activity.
