The employer did not ensure that eye and face protection was worn by affected employees to protect against flying particles, on or about 3/1/2023.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 219 Valley Rd, Clifton, NJ 07013: Employees engaged in roofing activities on a 8 on 12 pitch roof were exposed to fall hazards of approximately 15ft to the ground below in which personal fall arrest harnesses were not being utilized and not secured to an anchor point to prevent fall hazards, observed on or about 07/23/2024.

29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:  A) 219 Valley Rd, Clifton, NJ 07013: Employees were exposed to fall hazards of approximately 18 feet while using ladders to access a roof landing. The ladders that were used, did not extend at least 3 feet above the landing, on or about July 23, 2024.
