29 CFR 1910.106(h)(7)(ii)(b): Hot work, such as welding or cutting operations, use of spark-producing power tools, and chipping operations shall be permitted only under supervision of an individual in responsible charge who shall make an inspection of the area to be sure that it is safe for the work to be done and that safe procedures will be followed for the work specified. a) On or about 06/27/2024; in the Making Department, safe procedures such as the "Spark Hazard Permit Procedures" were not followed when pumping a flammable mixture of Carbon disulfide (CS2) and water with a submersible pump in a Class I hazardous location, exposing employees to fire and explosion hazards. When the water pump suffered a gasket failure, water leaked inside the cabinet causing a fire. Employees were exposed to smoke inhalation. NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) On or about 2024 in the Reclamation area; containers of a flammable mixture of Carbon Disulfide (CS2) and water did not have labels that included appropriate hazard warnings such as, but not limited to: flammability rating of 3 and a Health Hazard of 2. Since containers of the mix of CS2 and water were wrongly labeled there were inappropriately disposed causing a fire. Employees were exposed to explosion and fire hazards. ABATEMENT DOCUMENTATION REQUIRED
