29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) Location: Jobsite- 43 Barnes Blvd, Hudson, MA Frequent and regular inspections were not conducted adequately to identify and correct worker exposure to fall hazards where fall protection was not utilized and to identify and correct worker exposure to fall hazards and crushed-by hazards when a worker was standing on an unsecured load of plywood that was lifted approximately 25 feet above ground level by a CAT TL1055C Telehandler to conduct framing activities
