29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: a) 21 Steel Road South Morrisville, PA 19067- On or about July 24, 2024, employees working in the blast shed of the facility were exposed to slip and trip hazards due to an excessive amount of blasting agent (Black Beauty) accumulating on the floor.

29 CFR 1910.134(h)(2)(i):Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: a) 21 Steel Road South Morrisville, PA 19067- On or about July 24, 2024, a Bullard 88VX Supplied Air Respirator was left on a wall of the blast shed in a pile of Black Beauty, exposing it to damage and contamination.
