On or about July 6, 2024, at an establishment located at 921 South 10th Street in Manitowoc, Wisconsin; the employer did not provide a safe means of access and egress to and from each walking-working surface. Employees entering a storage closet from the kitchen were exposed to trip and fall hazards when the cellar hatch door in the floor of the storage room was not closed prior to entry.
