On or about July 6, 2024, at an establishment located at 921 South 10th Street in Manitowoc, Wisconsin; the employer did not provide a safe means of access and egress to and from each walking-working surface. Employees entering a storage closet from the kitchen were exposed to trip and fall hazards when the cellar hatch door in the floor of the storage room was not closed prior to entry.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  On or about July 24, 2024, at an establishment located at 921 South 10th Street in Manitowoc, Wisconsin; in the storage closet on the north wall of the kitchen above the door, a junction box used to power the camera overlooking the kitchen was missing a cover, exposing employees to electrical shock hazards.
