29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Jobsite located at 1643 N 6th St. in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer did not initiate or maintain an accident prevention program to address the hazards that employees are exposed to during framing activities, such as but not limited to fall hazards.

29 CFR 1926.503(a)(1): Employer did not provide fall protection training to those employees who might be exposed to a fall hazard:  a) Jobsite located at 1643 N 6th St. in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer did not provide fall protection training to employees who were exposed to fall hazards.  Employees performed framing activities without being trained on fall protection, thereby exposing employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) Jobsite located at 1643 N 6th St. in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer did not ensure an employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system.  Employees were framing the residential building approximately 12 feet above the ground level without the use of a fall protection system, thereby exposing employees to fall hazards.  b) Jobsite located at 1643 N 6th St. in Philadelphia, Pennsylvania- On or about July 25, 2024, the employer did not ensure an employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system.  An employee was framing the pilot house of residential building approximately 10 feet above the lower level without the use of a fall protection system, thereby exposing employees to fall hazards.
