29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  (a) Line 4 - Case Packer; On or about July 25, 2024, and dates prior, directly behind the operator's seat was a projecting, rotating shaft end that was approx. two inches in length, one and a quarter inches in width, and had two thread inserts that were not flush with the shaft sleeve.  (b) Production floor - Assembly Line 2; On or about July 24, 2024, and dates prior, various projecting, rotating shaft ends on the conveyor system contained collars that did not present a smooth edge.
