29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.  On or about July 25, 2024, and August 6, 2024, employees were not trained on hazardous energy sources. Employees did not have the knowledge, skills, required for the safe application, usage and removal of the energy sources while performing servicing and maintenance, therefore, employees were exposed to crushing hazards while placing their hand(s) under the die to feed material and make adjustments to the spacers/screws without de-energizing following machines.  a. Minster mechanical power press 2 Line 6 b. Minster mechanical power press 2 Line 12   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about August 6, 2024, an employee was exposed to unguarded nip points on but not limited to the H-11 press brake between the ram and the die when the machine is in operation.   b. An employee is exposed to a nip point while feeding the wire and adjusting it throughout the shift on mechanical power press 2 Line 6 . The wire is fed into the curler on the machine and then closes and completes the hinge.  No abatement or documentation is required for this citation.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es).  On or about August 6, 2024, and thereafter employees were exposed to but not limited to nip points and pinch points on the following mechanical power presses:   a. Employees were exposed to pinch points while the press was in operation due to there were no safeguards installed around the dies on the Minster mechanical power press 2 line 6. b. Employees were exposed to pinch points while the press was in operation due to there were no safeguards installed around the dies on the Minister mechanical power press 2 line 12.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
