29 CFR 1910.269(c)(3)(ii): Additional job briefings were not held when significant changes occurred during the course of the work, affecting the safety of the employees: (a) On or before 7/24/24, the employer did not ensure that an additional job briefing was held when the crews noticed work that needed to be performed outside the original scope of work. After completing the original scope of work and re-energizing the pole, the crew began securing a loose pole ground without re-assessing or amending the plan of how to safely accomplish the task. This condition exposed employees to electric shock and electrocution hazards.

29 CFR 1910.269(a)(2)(i)(C): The employer did not ensure that the degree of training was determined by the risk to the employee for the hazard involved: (a) On or before 7/24/24, the employer did not ensure that employees were trained in methods and procedures to understand the best methods to be out of the minimum approach distance to 13.2KV conductors. Employees were securing a pole ground inside the minimum approach distance and were not using one or more methods of insulating themselves from the energized parts or de-energizing the parts adjacent to the pole ground they needed to secure. An employee was shocked and burned when stapling the pole ground while wearing leather work gloves. This condition exposed employees to electric shock and electrocution hazards.
