29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) Mechanic shop, Bay 3 - On or about June 19, 2024, an employee was using the valve truck air springs to lift the frame off the rear leaf springs of a 2009 Freightliner 24-ft box truck when the driver's side air spring became overinflated and exploded. The employer not have a LOTO program.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:   (a) Mechanic shop, Bay 3 - On or about June 19, 2024, an employee was using the valve truck air springs to lift the frame off the rear leaf springs of a 2009 Freightliner 24-ft box truck when the driver's side air spring became overinflated and exploded. The employer did not have lock out tag out procedures to control the truck air springs.

The employee was not trained on the purpose and use of energy control procedures.

The establishment employed 71 employees during calendar year 2023  and is classified 484210 during calendar year 2023.  Note: Employer must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.
