29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. Location: a) Worksite; 4th and 5th floor; Building B; Guardrails facing north towards Tilden Ave and along Bedford Ave a) On or about 7/26/2024, an employee climbed over the guardrails, which had a break in elevation of approximately 47 inches, to access the formwork on the north side of the building without the use of stairways or ladders.
