On or about July 26, 2024, at the above addressed jobsite, employees using ladder Vader below employees installing weather guard were not wearing a hard hat to protect against falling materials.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. On or about July 26, 2024, at the above addressed jobsite, employees performing residential roofing operations, fifteen (15') feet above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Koster Roofing Co. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1702218, citation number 2, item number 1 and was affirmed as a final order on November 28, 2023, with respect to a workplace located at 7600 Stonecrop Ln., Joliet, IL 60431.  Koster Roofing Co. was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1566175, citation number 1, item number 1 and was affirmed as a final order on March 2, 2022, with respect to a workplace located at 20609 S. Frankfort Square Rd., Frankfort, IL 60423.
