29 CFR 1926.57(f)(5)(i): Employers did not use only respirators approved by NIOSH under 42 CFR part 84 for protecting employees from dusts produced during abrasive-blasting operations:   a) Location:  Mountain Gate Apartments - 7501 S. Utica Dr., Littleton, CO 80128.  On or about July 16, 2024, and at times prior, the employer, Centennial Concrete & Waterproofing LLC, did not require the use of Type CE Supplied Air Respirators when employees were required to engage in abrasive blasting operations using steel shot abrasive media to abrade approximately 464 square feet of concrete floor surface, in the enclosed breezeways and landings of Building 19.  The employer required employees to wear air-purifying respirators with N95 filters and safety glasses. This condition exposed employees to respirable crystalline silica dust and eye injuries from rebounding steel shot abrasive media and debris.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1:  a) Location:  Mountain Gate Apartments - 7501 S. Utica Dr., Littleton, CO 80128.  On or about July 16, 2024, and at times prior, the employer, Centennial Concrete & Waterproofing LLC, did not ensure engineering controls listed in Table 1 were implemented during dry concrete grinding operations for uses other than mortar removal and for a working time period of greater than 4 hours.  Employees were required to grind dry concrete surfaces in an enclosed area with a 7-inch Makita powered grinder.   The employer did not ensure the Makita grinder was equipped with a grinding wheel or blade shroud as well as a dust collection system that provided 25 cubic feet per minute (cfm) or greater of airflow per inch of grinding wheel diameter, a filter with 99% or greater efficiency, and a cyclonic pre-separator or filter-cleaning mechanism.  This condition exposed employees to respirable crystalline silica.  Centennial Concrete & Waterproofing LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA Inspection 1705321, Citation 1, Item 3a, which was affirmed as a final order on May 2, 2024, with respect to the workplace located at 5105 DTC Pkwy. Greenwood Village, CO 80111.
