29 CFR 1910.213(c)(1): Circular handfed ripsaws were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: a)Rosebud Wood Products, Inc. at 701 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to July 26, 2024, the employer did not provide a properly adjusted and maintained blade guard on the SawStop table saw, Model ICS53230, located in the southeast corner of the facility. This condition exposed employees to amputation hazards. b)Rosebud Wood Products, Inc. at 701 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to July 24, 2024, the employer did not ensure that the blade guard utilized on the Altendorf sliding table saw Model WA8 adequately covered and protected employees from coming into contact with the blade during operation. This condition exposed employees to amputation hazards.

29 CFR 1910.213(c)(3): Hand-fed ripsaws did not have non kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator: a)Rosebud Wood Products, Inc. at 701 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to July 26, 2024, the employer did not ensure that antikick-back devices were equipped during rip cutting on the SawStop table saw, Model ICS53230 located in the southeast corner of the Fabrication area in the facility. This condition exposed employees to amputation hazards and struck-by hazards. (b)Rosebud Wood Products, Inc. at 701 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to July 24, 2024, the employer did not ensure that antikick-back devices were equipped during rip cutting on the Altendorf table saw Model WA8 located in the central Panel Room of the facility. This condition exposed employees to amputation hazards and struck-by hazards.

This condition exposed employees to amputation hazards. b)Rosebud Wood Products, Inc. at 701 Southeast 12th Street, Madison, SD 57042: On or about and at times prior to July 26, 2024, the employer did not ensure that the Delta 14 inch bandsaw, serial number 94C99661 located in the Assembly area or Northwest corner of the cabinet shop was adequately guarded in that approximately 7.5 inches of the non-working portion of the blade was exposed during operations. This condition exposed employees to amputation hazards.

Employees work with woodworking and Computer Numerical Control (CNC) equipment that involves servicing and maintenance and were not provided instruction involving the use of the purpose and use of the energy control procedures. This condition exposed employees to lacerations hazards, caught-in hazards, and amputation hazards.
