29 CFR 1910.178(n)(4):Industrial truck driver(s) were not required to slow down and sound the horn at cross aisles and wherever vision was obstructed. On or about July 23, 2024, employees engaged in material handling and manufacturing activities in the facility located at 4641 North 56th Street, Lincoln, NE 68504, were exposed to struck-by hazards in that employees were operating powered industrial trucks in close proximity to other powered industrial trucks, as well as near pedestrian foot traffic, all without using the horn of the powered industrial truck as an audio warning while operating in areas where vision is obstructed to include but not limited to: aisleways, corners and near stacked material
