29 CFR  1910.151(c): Where the employer provided employees with an emergency eyewash station, the eyewash station was not maintained in proper working condition:    (a) Production work area, Column W-2: On or about July 25, 2024, the eyewash station located adjacent to column W-2 was not maintained in proper working order in that the supplied water pressure was not maintained at a level to be non-injurious  to an employee user.  NOTE 1: Proper maintenance and weekly testing is necessary to ensure that Emergency Drench Showers and Eyewash Stations are functioning safely and properly. Weekly testing helps clear the supply lines of sediment and bacteria build-up that is caused from stagnant water. The ANSI standard states that plumbed flushing equipment, "shall be activated weekly for a period long enough to verify operation and ensure that flushing fluid is available".  Furthermore, the ANSI Z358.1-2009 standard also requires Portable and Self Contained equipment "be visually checked to determine if flushing fluid needs to be changed or supplemented". (ANSI Z358.1-2009)

29 CFR  1910.303(b)(7)(i):Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  (a) Production work area, Column W-2: On or about July 25, 2024, employees are exposed to an electric shock hazard due to a missing "knockout" on the Square D electrical disconnect/junction box located on column W-2.
