29 CFR 1910.28(b)(3)(v): 29 CFR 1910.28(b)(3)(v): The employer did not ensure that each employee is protected from falling through a hatchway and chute floor hole by one of the methods listed in 29 CFR 1910.28(b)(v)(A)-(C): a) Reel Area, on or about July 15, 2024, and at times prior to the employer exposed employees to fall hazards of approximately 9 feet in that the employer required maintenance employees to work in the J1 reel area in close proximity to an open chute, referred to as the "Broke Hole," without adequate fall protection.
