29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On July 9, 2024, at 11929 W. 66th Terr. Shawnee KS, employees were exposed to falls of approximately 18.4 feet from the eave of the roof to the ground below.

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. On July 9, 2024, at 11929 W. 66th Terr. Shawnee KS, employees were exposed to fall hazards of approximately8 feet struck by not having the Warner adjustable ladder properly adjusted to the required 3' height above the landing surface.
