29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or before July 31, 2024, and at times prior, the exposing employer did not ensure that employees were protected from injury, in that several employees removing shingles and other roofing material on the roof of a one-story home were not utilizing a fall protection system. This condition exposed employees to falls greater than 10 feet.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid: a) On and before July 31, 2024, and at times before, the employer did not ensure that a 13-foot metal portable extension ladder, used to access the roof of the single-story residential house, had side rails extending 3 feet above the upper landing, and was secured. This condition exposed several employees to fall hazards greater than of 12 feet.
