29 CFR  1910.147(a)(3)(i): The employer failed to establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees.   Location: 198 Quincy St, Dorchester, MA 02121  On or about 07/31/2024, the employer did not have a hazardous energy control program with procedures to prevent the unexpected energization, startup or release of stored energy during servicing or maintenance of the mixer.
