The forklift operators received hazard awareness training but did not receive refresher?forklift operator?training before operating a forklift again.

29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged: a) Home City Ice/ Glacier Premium Ice Warehouse - On or about July 22, 2024, the employer failed to make sure loads handled in tandem by the Versa-Lift 25/35 forklift (SN# 02962/with boom extension) and Hyster 155XL2 forklift (SN# B024V01667Z/ with boom extension), were stable and safely arranged while lifting a fifty (50) feet long by eight (8) feet wide and one (1) foot thick floor panel, thereby exposing employees to a caught in-between hazard.
