29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person:    (a) On or about July 31, 2024, at or near 1037 & 1043 60th Avenue South in West Fargo, North Dakota, an employee was exposed to fall hazards of over 10 feet while installing wall sheathing, from a platform raised by a telehandler were the forks only extended four feet into a platform of over nine feet in length. The scaffold was not designed by a qualified person.
