29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a)Heavy Constructors, Inc. at Anamosa Street Bridge and West Boulevard North, Rapid City, SD 57701: On or about and at times prior to August 1, 2024, the employer did not ensure that employees conducting storm sewer drain installation in an excavation greater than five feet in depth were protected from cave-in hazards when working outside of a trench box. This condition exposed employees to cave-in and struck-by hazards.
