29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Along the roadway near 1817 Hammond Ave.; On or about July 31, 2024, the employer did not ensure that an employee working in an excavation with an approximate maximum depth of 7-feet, 6-inches and sidewall sloping ranging between 55 to 80 degrees, to install temporary water service, was provided with a proper means of egress from the excavation.

29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: (a) Along the roadway near 1817 Hammond Ave.; On or about July 31, 2024, a paved roadway running along the top east edge of an excavation, with an approximate maximum depth of 7-feet, 6-inches, was undermined up to approximately 18-inches under the edge of the road and was not supported to prevent possible collapse. An employee was working within the excavation to install temporary water service.
