1926.1060(a)(1)(iii) The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:(a) a) On or about July 31, 2024, at the jobsite: the employer's ladder was not extended 3 feet passed the roof landing, it was set up approximately 30" inches above the upper landing surface, exposing employees to 9 feet-2 inches fall hazard.
