29 CFR 1910.23(c)(9): The employer did not ensure that portable ladders that are used on slippery surfaces are secured and stabilized: On or about July 27th, 2024, employees who used a ladder in the Snow Reel Room at the Arctic Glacier facility at 8211 F Street, Omaha, Nebraska were exposed to fall hazards in that the floor is covered with ice and the ladder is unsecure and unstable.

29 CFR 1910.23(b)(13) The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. On or about July 27th, 2024, employees who used a ladder in the Snow Reel Room at the Arctic Glacier facility at 8211 F Street, Omaha, Nebraska were exposed to fall hazards in that employees carry a broom while climbing up and down the ladder, that is used to sweep out the Snow Reel and it could cause them to lose their balance and fall.
