29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    A).	On or about 08/01/2024, at the site of 221-09 Corbett Road. Bayside, NY  Employees fixing water leakage on a 6 and 12 pitch roof were not protected from falling approximately 22-feet by any type of fall protection system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 ft (0.9 m) above the upper landing surface to which the ladder was used to gain access:  A) On or about 08/01/2024, at the site of 221-09 Corbett Road. Bayside, NY  Employees were using a portable ladder to gain access to a roof. The ladder side rails extended approximately 1 1/2 feet above the upper landing surface. The employees were exposed to the hazard of falling approximately 22 ft to the ground below.
