29 CFR  1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces were corrected or repaired before an employee used the walking-working surface again. When a correction or repair could not be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard was corrected or repaired:   (a) On or about August 12, 2024, employees were not protected from being struck-by steel parts due to a partial loss-of-load from powered industrial trucks required to operate on the concrete floor of the plant that had potholes on it which were not repaired or guarded.  (b) On or about August 12, 2024, employees were not protected from tripping while required to walk and work on the concrete floor within the plant that had potholes on it which were not repaired or guarded.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about August 12, 2024, the back of the shear press that is part of Gogan Line 1, known as TG1, was unguarded in that the door to the safety cage it is in was locked and remained in the open position, so employees were not protected from the machines point of operation.  (b) On or about July 31, 2024; the door of the safety cage to manual eye machine/eye forming machine 2, known as EM2, was unguarded in that the chain for the quick disconnect broke off and the machine did not turn off when employees entered the safety cage, so employees were not protected from the machines point of operation.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible.  Machine guard(s) on machine(s) were so installed that they offered accident hazards in themselves:  (a)  On or about July 31, 2024, the guard on Shear machine 1 offered an accident hazard in itself in that it extended out over the bin that the machine drops parts into. Operators are required to reach down into the bin to straighten the parts and can strike their head and/or upper body on the guard when doing so.  (b)  On or about July 31, 2024, employees were not protected from the chain and sprockets of the conveyor that feeds parts into the furnace on Gogan line 1 in that the guard was not affixed to the machine or secured elsewhere and was pushed back approximately 11 inches.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  On or about July 31, 2024, employees were not protected from electrical shock in that: (a)	On shear machine 2 (marked SM1) in the shear area, there was an electrical box with a cover, but it was not attached and electrical wiring powered at approximately 240 Volt was within it. (b)	On Gogan line 1, at the end of the frame to the squeezer/side taper machine, there was an electrical junction box with no cover on it and wiring in it was powered at approximately 120 Volt.

29 CFR 1910.305(j)(4)(v): The disconnecting means was not readily accessible:  On or about July 31, 2024, employees were not protected from dangerous machinery in that:  (a)	The taper roll machine (TG1) on Gogan line 1 did not have a readily accessible disconnecting means because it was mounted to the southwest wall behind the machine and not the front of the machine where the operator stood. (b)	The squeezer/side taper machine on Gogan line 1 did not have a readily accessible disconnecting means because it was mounted to the southwest wall behind the machine and not the front of the machine where the operator stood.
